 person.\nRegistration is $250 plus a $25 concession fee that is  refunda
 ble ONLY when a full\, regular season game is worked by a  parent/guardi
 an during the current season. Fall registration  applications can be com
 pleted on-line or downloaded. Once you have  completed your registration
  application\, please print and sign. Please  make sure that you complet
 e the NGFL insurance waiver and GMSAA forms  also\,as these forms are MA
 NDATORY for your child to play this season. **If  you completed the onli
 ne application for spring there is no need to  complete it again. You wi
 ll need your signed application\, GMSAA  insurance/ waiver form and a $1
 50 equipment deposit check to pick up  your players equipment. Payment O
